Program: Home Delivered Meals Program - Meals on Wheels by ACC

Description
Home-delivered meals (Meals on Wheels): Noon meals delivered five days a week to clients who are unable to prepare nutritious meals for themselves and have no other means of having meals prepared for them.
Prior to receiving home-delivered meals, an assessment worker will make a home visit to determine eligibility, explain the program, and provide information on topics such as fall prevention. Individuals receiving home-delivered meals are reassessed every six months.

Eligibility: | Seniors ages 60+ who are homebound and frail. No assistance available for those under 60 even if disabled. |
Languages: | Most languages by arrangement, English |

Program Fees: | Suggested contribution is $2 per day/per meal. |
